If you are a medical professional and would like to have patient samples sent to your office or clinic, please fill in the following form.
If you have a question about Cold-EEZE® not addressed in the FAQs section, please call 1-800-505-COLD (2653) between the hours of 9 AM and 5 PM EST. If this is a patient emergency, please leave your name and contact information and we will respond to you on the following business day.
The Spring 2009 Patient Sampling Program has expired. To be added to the mailing list for future patient sampling programs, please complete the form below and be sure to fill in ALL REQUIRED FIELDS. Patient samples are available only to professional medical practices. All sample requests will be verified prior to the shipment of samples. Thank you.
